Post-Hospital Rehabilitation

Unlike most post-hospital rehabilitation programs, we offer our patients specialized therapy offered by our on site therapy team. Our therapists are available up to 7 days/week and have the ability to work with the same patients during their rehabilitation stay to ensure continuity and positive outcomes.

It is our goal for our patients to succeed through treatment and return home to an independent life. Our resident-centered facility is staffed with nurses, therapists, and area physicians who will deliver quick recovery with the results you should expect. The focus of this program is to achieve the highest level of recovery through individually designed rehabilitation care plans created with our interdisciplinary care team.

Physical Therapy
- Focuses on endurance, improving strength, helps regain or improve balance mobility and ambulation.
Occupational Therapy
- Restores activities of daily life and includes feeding, dressing and bathing.
Speech Therapy
- Emphasis on regaining ability to swallow and speak while focusing on cognitive and communication skills.

Cardiac Care

Program Features

Management of patients with Congestive Heart Failure, Post-Coronary Artery Bypass Surgery, Myocardial Infarction (heart attack), and LVAD’s
Consultations by a Board Certified Cardiologist
Speciality-trained staff in cardiac diseases
On-site coumadin management, labs & diagnostic testing
Physical, Occupational & Speech Therapies available up to 7 Days per Week

Pulmonary Rehabilitation & Recovery

Program Features:

Protocols for the management of Chronic Obstructive Pulmonary Disease (COPD), Pneumonia, Emphysema, Asthma and other respiratory diseases
Consultations by a Board Certified Pulmonologist
Evaluation & Treatment by a certified Respiratory Therapist
Patient and staff education on managing pulmonary disease and symptoms
Functional performance assessments & Pulmonary function testing
Managing AVAP’s on-site with ongoing education and training for staff and therapists
Physical, Occupational & Speech Therapies available up to 7 Days per Week

Renal Management

Program Features:

In-House Hemo Dialysis
Specialty-trained dialysis staff
Consultations by a Board Certified Nephrologist
Individual dialysis machines
Nutritionist-supervised dietary program
Working in collaboration with outpatient dialysis treatment providers

Wound Management Program

Program Features:

Designated certified wound care nurse
Treatment of vascular, diabetic, pressure & surgical wounds
On-site Wound Physician & Infectious Disease Specialist – consultations as needed
Staff education focused on wound etiology and management to heighten awareness and surveillance
Individualized treatment plans and daily consultations with the interdisciplinary team
Wound Vac management
Dietitian rounds for nutritional needs assessment to promote healthy wound healing
Antibiotic Management
Family education and communication on status, progress, and care

Chicagoland's Premier Therapy Provider

Ability Rehab

Post-Hospital Rehabilitation patients will receive on site, specialized therapy offered by our contracted therapy provider, Ability Rehab. Ability Rehab is Chicagoland’s premier contracted provider of Physical, Occupational and Speech-Language rehabilitation. Ability therapists are available up to 7 days/week and have the ability to work with the same patients during their rehabilitation stay to ensure powerful therapy solutions and optimal patient outcomes.

Ability Rehab has a mission to provide patient-centered care to ensure optimal outcomes and increase overall success. It is our goal for our patients to succeed through treatment and return home to an independent life. Our resident-centered facility is staffed with nurses, therapists, and area physicians who deliver quick recovery with the results you should expect. The focus of this program is to achieve the highest level of recovery through individually-designed rehabilitation care plans created with our interdisciplinary care team and Ability therapists – a therapy provider you can trust.

Long-Term Care

At all of our locations, you will find our long-term care is completely customized to our residents’ needs. This is because we begin with a comprehensive assessment which is completed and reviewed by our comprehensive interdisciplinary team which includes nursing, physical/mental therapy, nutritionists, as well as activities specialists and social services professionals. The result? Your loved one will have a customized healthcare plan designed to optimize his or her quality of life.

Memory Care

Meadowbrook offers a safe and secure environment for individuals with Alzheimer’s or Dementia. Our unit boasts a variety of activities, specially-trained staff, and a sense of comfort you will grow to rely on.
