This enhanced on-site specialty care initiative expands access to comprehensive cardiac services for residents living with complex heart conditions, reinforcing Meadowbrook’s commitment to quality outcomes, continuity of care, and reduced hospitalizations.
Branded under the guiding principle “Specialty Care. Proven Results.”, the program brings advanced cardiac oversight directly to residents through on-site specialists, diagnostics, and personalized care planning. By integrating Alteas Health’s clinical expertise within the Meadowbrook care setting, residents benefit from proactive management, early detection of clinical changes, and seamless coordination across care teams.

The Specialty Cardiac Care Program supports a wide range of cardiac conditions, including arrhythmias, atrial fibrillation and flutter, coronary artery disease, congestive heart failure, cardiomyopathy, hypertension and hypotension, post-CABG surgical follow-up, pacemaker and LVAD management, peripheral vascular disease, and more. Residents also have access to cardiac clearance services, chest pain evaluations, and monitoring for advanced cardiac devices such as Life Vests.
Key benefits of the program include early detection and proactive management of cardiac symptoms, direct specialist oversight for complex conditions, and seamless continuity of care following hospital-based interventions. On-site diagnostic capabilities—such as EKG, telemetry, spirometry, and sleep studies—help reduce delays in care, minimize unnecessary transfers, and support faster clinical decision-making.
